SUBMISSION REQUIREMENTS

Submit:
 Email the nomination package (Click here to download forms) to AWARDS@CPFEA.US

WHAT DO I NEED TO SUBMIT? Below are the components of the nomination package. We have made it simple by including a Word version of the application form for your use. 

 

  • Nomination Cover Page – use the blank form

 

  • Nomination Form – Use the blank form

 

  • Award Citation – The citation should be a “brief” synopsis (approximately 75 to 100 words) of the major accomplishments of the nominee. Specific examples from the nomination package should be cited.  The citation will be read as the winner is announced. Use the blank form

 

  • Performance awards, suggestion awards, and military decorations or honors may be considered and included in the narrative.

 

  • Note: Receipt of highest performance rating is not a prerequisite for the nomination.

 

IMPORTANT REMINDERS THIS YEAR

  • Nominations must be received by Sunday, August 20, 2017, at 3:00 p.m.
